When completing a new passport application, you have three options: obtaining a form from a local post office, downloading one online, or using an online form filler.
If you encounter technical difficulties while using the Form Filler, we recommend downloading a printable PDF and completing the form manually, and handwriting it with black ink.
Use the U.S. Department of State’s online passport application wizard to create a 2D barcode application form in PDF format. Most acceptance agents require the DS-11 form to be barcoded.
The DS-11 form is used for new passports and child passports, while the DS-82 is used to renew expired passports.
While the DS-11 form can be printed in black and white or color, it must be filled out using black ink.
Do not print your form horizontally. Instead, print your form single-sided on letter-sized paper (8.5″ x 11″). The State Department does not accept double-sided forms.

Form DS-11 Form requires photos to be affixed by stapling or gluing. Staples should be placed away from the applicant’s face.
The average processing time for application form DS-11 takes about 6 to 8 weeks. The United States Expedited processing takes about 2 to 3 weeks to complete.
